To answer this question, first look at the map in the Individual Notice on the Home Page. If you still cannot tell, please contact us at 1-800-345-0837, and give us your address between January 16 and January 19, 2007. We will look it up and try to help you make the determination.
2. How do I get help in completing my claim form?
Call us at our toll free number, 1-800-345-0837, and we will be happy to help you complete your claim form.
3. When is my claim form due?
You must mail us your completed claim form no later than November 28, 2008. Our address is: Settlement Administrator, P.O. Box 12327, Birmingham, Alabama 35203. Please note that you must send us either a completed red claim form for ordinary damages or a completed blue claim form for extraordinary damages. You cannot send us both.
